Medication Therapy Management is the analytical, consultative, educational, and monitoring services provided by pharmacists in order to facilitate the achievement of positive therapeutic and economic results from medication therapy.
The MTM program provides up to 30 minutes of face-to-face consultation with our pharmacist to review your medications. Our pharmacist will work closely with you, your doctor(s), and your family to understand the issues and concerns you face with your medications and your health – and help you to feel your best by getting the most out of your medications.
Our pharmacist may help you:
- Understand your medications and answer any questions about your medicine or medical conditions.
- Check for drug interactions between your medications.
- Reduce possible side effects from your medications.
- Organize your medications to help decrease side effects.
- Find less expensive medication alternatives.
- Simplify your medication regimen.
- Monitor your medications and medical conditions (such as high blood pressure, high cholesterol, or diabetes) to be sure you’re getting the most out of your medications.
This service will not interfere with the physician-patient relationship. We are here to help physicians and their patients with Medication Therapy Management.
The full cost of this service is covered by your plan. There are no co-payments, co-insurance, or deductibles to meet.
Ricker Pharmacy is happy to offer Medication Therapy Management (MTM) to help patients with their every medication need. This program is designed to enable pharmacists to work in collaboration with local prescribers to improve our patients’ care.
As part of MTM, pharmacists may do an extensive medication review of a patient’s medications, as well as herbal products, vitamins, minerals, and over-the-counter therapies. Also, we will get in touch with patients for follow-up monitoring visits, when needed, to evaluate their progress with new medications, and to guarantee that their drug therapy issues have been resolved.
Our MTM program is designed to help both you and your patients and offers these benefits:
- Safer and more effective medication use. We believe that teaching patients correct medication use can enhance their adherence, help them attain and manage their therapeutic goals, and avoid medication errors. To guarantee medication effectiveness, our pharmacists will do follow-up phone calls to keep track of a patient’s symptoms and adherence to the medication regimen.
- Selection of the most therapeutic and cost-effective medications. We provide patient counseling services that can help in choosing the most cost-effective and clinically appropriate generic or brand-name medications. If we discover a medication that is more cost-effective than your current medication, our pharmacists will consult you and obtain your authorization before any medication changes take place.
- Improved coordination of care. Many patients see different providers for various health conditions, which may result in duplicate therapies or harmful medication interactions. We will give our patients a master medication list that has information related to all of their medications, and a medication action plan that describes the safest and most efficient use of their medications. We will encourage patients to bring these records on all visits with you and other healthcare providers.
- MTM Service documentation. Our pharmacists will meticulously document all MTM services offered to eligible patients. In addition, you will be informed if a patient’s clinical situation requires your attention. Any changes made to a prescription medication will only be done with your prior authorization. All documentation will be stored at our pharmacy and will be available to you when needed.
